When you see a Credentialled Diabetes Educator and Nurse Practitioner you can expect to receive personalised care that is based on your goals and needs.
Your First Visit
At your first visit we get to know each other. You can expect to be asked lots of questions so that a thorough assessment of our diabetes and general health needs can be determined.
To make the process easier we suggest you bring the following:
- Your referral
This is needed if you wish to access rebates from Medicare or DVA
Copies of recent blood tests are always helpful - Your Diabetes Equipment which may include:
Blood Glucose Meter and diary
All Medication (oral and injectable) or a list of your medications
A list of your questions and the goals for your visit
Your injection devices – if you take insulin or another diabetes injectable
Any diabetes technology (insulin pump, CGM or similar)
At the end of the first visit we will discuss a plan for your diabetes that focusses on your personal goals and needs. This may include future appointments and plans for specialist services.
